The Doctors & the Cheap medias!

[PS: - I am not directly or indirectly associated with any news channel/print media/political party/student union etc. There is no conflict of interest while framing this article]

Some say doctors are second only to god, some say doctors are the noblest professionals. For some, the
above beliefs may not hold true all the time & the reason is amalgamation of variety of factors like; quality of treatment, behavior of doctors, work load etc. Because of the diversity in working atmosphere & payment structure, we shouldn’t expect every doctor to be second only to god. Frankly speaking, no doctor wants that tag too. It doesn’t matter whether you are in cricket field or in Operation Theater; you would be either over-praised or over-criticized. We, the Indians are emotionally extreme. We can worship & kick the same person in different situations. You can well visualize the situation in the scenes of old Hindi movies where the actors scold the God while facing crisis but deep within they tend to carry limitless devotion. The same thing happens in real life of doctors. People will praise you when you successfully remove gall bladder in 99% of the occasions, but they will voluntarily & seriously defame you if you injure the bile duct in 1% of the cholecystectomy surgeries. One single mistake turns the God into a Dog!

Mistakes are unavoidable byproducts available free of cost in all types of professions. But the
mistakes committed by the health professionals get the unwanted limelight because they are in direct contact with society. If a software engineer commits a mistake while programming something, it never affects the general population directly. On the other hand, doctors don’t get the privilege to rectify the mistakes they commit. So these factors project a pseudo-magnification of a doctor’s professional/personal mistakes. Now coming to the term ‘Cheap media’, I admit that I have developed life threatening allergy to them. These are the ultra-low profile TV news channels & news papers owned by some of the illiterate & tadpole-brained politicians. Government hospitals are bread& butter for some of these Medias. Their journalists are well aware of the fact that people are highly sensitive towards news related to health professionals. Believe it or not, in >90% of the educated Indian families you should find someone who has prepared for medical entrance exam & >90% of them don’t get through. So I don’t rule out the jealousy factor out of my equation. The cheap medias always take this advantage by negatively exaggerating a news related to doctors. When the relatives of a patient beat a doctor for a wrong reason, it is never telecasted/ printed in these channels/news papers. But when the reverse happens for self defense, the reporters run faster than Usain Bolt to publish the news. There is nothing called ‘Self Defense’ in a doctors life! Nobody realizes the amount of workload & stress a doctor has to handle in real life. Guess what is the biggest joke by the judiciary system? It is the act that if someone beats a doctor on duty, there will be non-billable arrest warrant against the person. I have never seen/heard it being implemented anywhere in India.

I am not saying that doctors are universally innocent. But the way these cheap medias are projecting doctors makes me feel like being a criminal. Even the relatives & friends of cameramen/drivers/sweepers/peons of the news agencies show their red eyes to doctors while coming to hospitals. Unfortunately there is no media coverage for the red eyes. It is hard for the doctors to remain tight-lipped when these kind of people unnecessarily abuse in the stressful working atmosphere. So it is time for the people associated to TV/print media to have some introspection. They should recheck their credentials before prematurely abusing doctors without any evidence. What our health system badly requires now is a healthy relationship between the doctors & the patients; it will be great if the media can act as a vital cog in the wheel of the relationship.

[If you are related to TV/print media & you find this article offensive, then I am NOT AT ALL SORRY. Because I am writing my heart out & it feels true]

HAVE I MADE IT LARGE?

3-4 movies per day, end less gossips with friends on the terrace, over-use of social networking sites.. This has been the story since last 1.5 months. Currently all these entertainments are sandwiched by the pleasant hope of using the prefix 'Dr' & a frightening anticipation of getting failed in the exam. So the life is all about killing the boredom since the last the day of final MBBS exam. Few days back, all those text books (Harri+David+Ander+Nel)Sons were killing our time... but now we are searching for different methods apart from the books. I bet, for most of the people on the planet social networks are the best way to get relief from the  boredom & I am not the lucky exception. With the down falling falling of Orkut, most of the grey matters are jammed at Facebook & Twitter now a days. OK I am Sorry if I am confusing you with the title 'Have I made it large?' Definitely I am not a brand ambassador ROYAL STAG ! Its just about the psychological reflections of those addicted to Face Book & Twitter.

''Young girl commits suicide after not getting enough 'LIKES' on the face book profile picture''..... ''boy throws acids on girl's face after the later denied his friend request in F.book''..... ''famous fashion designer is depressed because of not getting enough followers in Twitter''

I think we are definitely going to hear such news in TV channels/news papers(P.S-I am not counting 'INDIA TV' in the channel list). We are so much addicted to these things that it'll be simply unavoidable in near future. If someone says he/she doesn't post a picture/update just to get the 'LIKEs', he/she is the biggest liar on this planet. Almost of all of us are using these sites for self-publicity.. that is the truth ! No body will admit that he/she was not sad after not getting the 'likes & comments'. Being frank I would love to reveal that some of my friends have phoned me in mid night to click the LIKE option or give some praise-coated comments on their posts. That is purely & surely ridiculous. My heart wanted to block those persons in FB but brain made me realize that it is a normal phenomenon. Even the likes of Amitabh Bachchan & Sachin Tendulkar are using networking sites. Struggling actresses like Minisha Lamba/Riya Sen/Antara Mali manage their self-created fan groups in Facebook. So it doesn't matter whether you are from a middle class family or you are the son/daughter of the Mallyas/Ambanis, you always crave for publicity. Shower of LIKEs & comments makes us feel that YES, I HAVE MADE IT LARGE !!!

One thing for sure, the social networking sites have given birth to millions of HYPOCRITES. Believe me, its really funny to go through the profiles of this type of people. As evident from the definition, they are the ones whose actions are not consistent with their claimed beliefs. 'I WHAT I AM'....'I AM AN OPEN BOOK' ...'I AM A SIMPLE GIRL' ... I BELIEVE IN SIMPLE LIVING, HIGH THINKING' ... These are the most common phrases in the 'about me' column in Facebook/Orkut/Twitter. They always try to convince how sweet/cute they are via these types of phrases. I have found few people to be completely opposite in their real life though their profile shows these phrase since they were zygotes. I believe this is just to balance/over-balance their social prestige. By this, the profile looks as if IT HAS MADE ITSELF LARGE !

According to a recent publication by the the Dept of human psychology of University of Cambridge, Social networks have drastically hampered the communication skills among teens. They usually get a false sense of satisfaction of being smart/aggressive/romantic in their Facebook profile. But in real life they struggle to catch up with their Facebook images which leads to depression, poor eye contact during communication etc. The researchers claim that it happens when the false sense of pride in virtual world is out of phase with the real world activities. So it is not all about MAKING IT LARGE in networking sites by funny updates & posting one of the good looking pictures(out of millions of bad looking pics).... its all about asking HAVE I MADE IT LARGE?? in the real life ...... ( my heart says the answer: NO NO NO )

[all of us have become the victims of these sites... frankly speaking, while typing this blog I was thinking whether people will LIKE this or not... whether the members of indiblogger.com will PROMOTE my post in indivine or not etc ... I realize, its a human instinct & its unavoidable]

NO I HAVEN'T MADE IT LARGE :(

GIVE ME ANOTHER STROKE

I always follow a principle: ‘NEVER SUPRESS YOUR EMOTIONS’… This really has been advantageous for me since last 3-4 years. In my opinion, writing is a great way to express emotions & as usual I am going to take its help. I had decided to say NO to blogging in the final year because of the tightly packed schedules, but one incident has made me so emotional that, I can no longer suppress my desire to blog. So I am feeling good to be back into this universe of bloggers & really enjoying typing something which is beyond my comfort zone (affairs/relationships)!!

1st November 2010.It was a fine, cool morning in Bhubaneswar & I was in the Apollo hospital for my project works. Let me say that, for a medico, no place is better than a tertiary care word class hospital like Apollo. I usually become consciously unconscious in the 1st 15-20 min after entering Apollo because the gorgeous receptionists in the centrally air-conditioned building make me feel like a business class passenger of a trans-continental flight. So after 20-30 min, I regain my composure & start my work. So the 1st November was not an exception. As a routine practice I had to be with my mentor, Dr Mishra during the rounds in the neurology indoor. During the rounds we entered room no- 448 in which an 80 year old widow was in bed & was staring at the ceiling. According to my mentor she was received in a state of coma 1 month ago, was diagnosed as a case of stroke & was treated accordingly. At that point of time the lady was fine & she was requesting to discharge her from the hospital. We were astonished when we didn’t find any of her relatives to talk about the discharge but Dr Mishra promised her that she’ll be discharged soon. After coming out of the room, he told me with a sad note that ‘I want to discharge her but her none of her bloody relatives are coming to the hospital’. Then we visited other indoor patients to complete the morning rounds & came back to his OPD chamber. (If you’ll compare the OPD chamber of Apollo to that of a govt. college you’ll feel as if the Apollo chamber is for Ema Watson & the Govt. chamber is for Rakhi Sawant).

After the indoor rounds my mentor attends the OPD patients & I do the neuro-assessments of all the stroke victims, which is very much similar to watching golf tournaments in TV i.e difficult & boring. After examining 2 patients I came to room no 448. Surprisingly I found that lady with tears in her eyes. But she behaved normally as if nothing has happened. Without asking anything I started the examinations & found her to be normal (believe me or not, everything appears normal to a normal medical student). But being driven by curiosity I asked her the reason of crying. First she denied, but then started crying again & told me with a trembling voice that, ‘I am fine since last 1 week, I want to go home but my sons are not taking me’. This was really one of the tear-dragging moments of my life. When I enquired more about her family, she told about her two sons, two daughters, daughter-in laws & the two son-in-laws who are doctors! Tears streaming down her eyes, she mentioned, ‘they are coming to the hospital only for 20-30min, just to complete the formalities. They are just checking whether I am alive or dead, none of them have appeared since last week’.  I was feeling very uneasy at that moment as I saw those things only in movies like Baghban & other TV serials but never in real life. Other than consoling her I could do nothing. LL
Then I returned to Dr Mishra to tell the whole story & to find out a solution.  After listening to these things, he was annoyed & looked more tensed than Viswanathan Anand in front of a chess board. He told me that during his 5 years in Boston he found the American sons to be more loyal to their parents as compared to the Indians. Then he asked me to search her son’s phone no from the patient file (that made me feel like the member of CID team in sony). After getting the number, he dialed with the speaker on. Here goes the conversation:
Dr-hey Mr X, your mother seems to be fine now, I want to discharge her.
Mr X- Thanks doc. But I am wondering if you can keep her for another 10-15 days in hospital.
Dr- but why? She wants to go home.
Mr X- Actually I have a 3 year old son & my wife at home. If she is discharged now, it’ll be difficult for my wife to manage the condition. My wife has asthma problem.
Dr- as you wish Mr X. but remember that she is emotionally fragile & it may lead to another stroke. Hope you understand.
Mr X-yes. I do understand but what can I do?? I am helpless. I’ll talk to you later.

Hearing these things, both of us were speechless. He told me, ‘we can do nothing in this case, just keep the old lady in your mind & promise yourself that you’ll never do that to your parents’ & started attending the patients. I too started my work but in the absence of brain & heart.  So many things jammed my grey matter.
Today is the auspicious Diwali & I am writing this story because I can’t forget the tears of the helpless mother. When I told the story to my parents, they told that they would have preferred to remain in coma in such condition because it is unbearable for any of the parents in the universe to receive such ridiculous behavior. She must have celebrated a number of diwalis with her family members but now no body cares what their mother feeling now in this diwali. She must have been worried about her children’s health in the past but now, nobody has time for her. A son has time to get worried about the asthma of his wife but not about the brain stroke of his mother!!  We haven’t grown up by our parents just to throw money for their treatment in modern hospitals; only thing they want from us in their old age is proper caring & affectionate behavior. So please keep your parents happy throughout their life because it’ll give you more happiness than getting a job in Microsoft or getting a seat in the Harvad/MIT, because our happiness is directly propotional to that of our parents'.

DOWN DOWN DOWN

3rd professional MBBS theory exams were over yesterday & I have a gap of 8 days for the brutal practical exams. I’d like to remind you the fact that getting 8 days break in MBBS life is really brain-soothing. We (normal students) do all types of work other than studies during short vacations & I am not an exception. Watching 3-4 movies at a stretch after the end of exam is a pleasant tradition in my college. Giving due respect to this tradition I watched movies & was engaged in social networking till 3AM. Almighty always gives good sleep to students just after the exams but in this part of world it wasn’t feasible. He/she reminded me that I’ve to attend the pediatric OPD for my incomplete project & smashed my fragile sleep into pieces. Nothing is more painful than when you had an incomplete & fragile sleep particularly when you slept so late in night. So my day begun like a typical Uday Chopra movie, unpleasant to the eyes, usually flop & DOWN.

It was 9.20 AM when my NIKE kissed the professor’s chamber in the pediatric OPD. The peon told me that Sir arrived sharply at 9.00AM which is the regular OPD opening time & this bitter truth tripled my heart rate. It is needless to describe how difficult it is to work with a punctual personality in India. ‘hey abhishek….you are 20 minutes late, what the hell you’re doing?’…Dr Modi’s tongue passed these words to my ears. He gave such an expression as if I had deliberately killed 20 patients in those inglorious 20 minutes. Dr Modi is my mentor for my research project & he is considered as one of the most student friendly pediatricians in my campus. But an angry expression from a cool person always appears lethal to a lethargic person like me. I just apologized politely & remained silent. Not surprisingly I was DOWN again.

The atmosphere in a pediatric OPD is always irritating especially when you are an insomniac. Lack of sleep, noise made by the crying babies & Dr Modi’s unusual angry looks were making me feel like an asthma patient & guess what. I was working on project related to asthma!! My eyes were desperately searching 2 things- a smile from my mentor & asthma patients. Finally I managed to get the usual smile on my favorite teacher’s face & which filtered out some of my stresses. I was waiting for asthmatics but god gave me something else!! It was 10.30 AM when a lady came to us with her 3 year old child & her mother-in-law. They were from a nearby village & probably from a family of low socio economic status.. The baby had a mongoloid appearance & it was diagnosed as a case of Down’s syndrome. God gave me another DOWN!! The baby’s grand mother told that they know it is down’s syndrome which pleasantly surprised me. It was refreshing to see rural Indian women talking about this disease but the half life of my happiness was too short. It was shorter than sahid afridi’s average innings duration because the old woman suddenly told that, ‘all these are happening just because of her mother. She must have some serious deficiency & she always brings bad luck to my family.’ These types of biased allegations from a family member brought tears in the innocent mother’s eyes. The cardio-toxic comments from the mother-in-law was unbearable for the lady She was helpless as were we. Suddenly sir broke his silence & scolded the old woman which finally terminated as typical medical counseling. ‘The mother is not the culprit & if there any one has to be blamed it should be god because it is a genetic disorder caused by a mutation. The baby is mentally retarded & needs proper care & guidance’. These words from Dr Modi soothed the mother & calmed down the furious grandmother. They went away after being counseled by him. Then sir told me that ‘we are apparently going to become one of the superpowers in the world but superstitions will never let us move forward, so spread as much awareness as you can regarding these diseases.’ So…what are you waiting for??? Let me do my duty.

Well, the proper counseling of the parents is always the best way. Babies with down’s syndrome are always mentally retarded which requires the parents to give all their attention opsonized with a lot of love & care. Cardiac disorders, gastrointestinal disorders, musculo-skeletal disorders, endocrinelogical abnormalities…..you just name it, they’ll get it in future. So they should undergo periodic medical check up regularly. There are a lot of associations working for these DOWN victims, so if possible, the parents should be encouraged to admit their babies in these centers. That’s all we can do for these innocent babies.… DIAGNOSE, COUNSEL & ENCOURAGE……

I hope I have not let you DOWN by writing about Down’s syndrome….

MESS IN A MESS

Do you have any other work other than writing against girls in your articles?’ This was the comment fired by one of my juniors ,Ms X. Undoubtedly it was a cardio-toxic comment for a sensitive guy like me, because this comment was pointing finger (middle one) towards my versatility in writing. Well, now my grey matters are accepting this comment with alacrity with the hope that it’ll inspire me to write something beyond my comfort zone (girls, romance, affairs etc.).So thanks to Ms X (no hard feelings for you dear). But what should I do Ms X??You’ll find the tag of error in each & every organism of this universe. Thank u for exposing my errors. So being a part of the psychiatric defence mechanism DISPLACEMENT I m going to expose one of the serious errors in the society.

I can’t understand what the govt of India is doing?? Why it can’t be prevented??Is poverty the only force which drags them into this profession??Some of the concerned officials frequently raid some low-profile hotels, Bars & other commercial establishments, which prevents their activity for a negligible period & the sad saga continues!!! Can you guess what I am talking about? Hey, dirty minds!!! I am not talking about PROSTITUTION. I am focusing on CHILD LABOR. Well, in some way these two are similar because of life-ruining properties of these deadly professions. The major difference is that, in the former, freshers are treated as angels. So  my first wish(1) there should be no child labourer in our society (2) if 1st wish doesn’t  get fulfilled,  they should be treated as angels as their counterparts in prostitution. OK, now give some stress to your optic nerve & have a look at the following data.

So, only 67.44% of the children within the age group 5-14 years attend the schools!! But according to the govt. of India formal education is compulsory & free of cost!! So what the hell is going on??There was a dialogue in the movie JANNAT -‘jab garibi darwaje se andar ati hai to pyaar khidki se bahar chala jata hai’’. Now you can replace the word ‘pyaar’ by EDUCATION-‘ Jab garibi darwaje se andar ati hai to EDUCATION khidki se bahar jata hai.’ According to me, poverty is a major factor that germinates child labour.
According to the Indian census of 1991, there are 11.28 million working children under the age of fourteen years in India.

o    Over 85% of this child labour is in the country's rural areas, working in agricultural activities such as fanning, livestock rearing, forestry and fisheries
o    The world’s highest number of working children is in India. ILO estimates that 218 million children were involved in child labour in 2004, of which 126 million were engaged in hazardous work.
o    Estimates from 2000 study suggest that 5.7 million were in forced or bonded labour, 1.8 million in prostitution and pornography and 1.2 million were victims of trafficking. In India, 1104 lakh children are working as labourers.

o    The Hindi belt, including Bihar, Madhya Pradesh, Rajasthan and Uttar Pradesh, account for 1.27 crore working children in the country, engaged in both hazardous and non-hazardous occupations and processes.
o    Over 19 lakh child labourers in the 5-14 age group are in Uttar Pradesh. Rajasthan accounts for over 12.6 lakh workers followed by Bihar with over 11 lakh and Madhya Pradesh with 10.6 lakh.
o    However, according to the 2001 census, in state-wise distribution of working children in the 5-14 age group, Andhra Pradesh with 13.6 lakh child labour stands second in the national list after UP.

Dipuna, Jitu, Siba, Jeevan, kalia etc!!!!!???  These are some of the familiar names in my hostel. They are neither gold medallists nor extremely bad students!!!! Then who are they???All of them are aged below 14 years, all of them are illiterate & all of them don’t enjoy any dream in their REM sleep. They are simple workers in our hostel mess who wash the used utensils & the dining tables. We really don’t care for them during lunch/dinner. We just shout at them to clean the tables, serve the food etc as if they were born only to serve us. Giving them 20-30 rupees during diwali/dussera never puts a full-stop to our job. I admit that, sometimes the quality of food in hostel is so irritating (rakhi sawant standard) that we forget to offer them a smile. They are doing so much for us from such small ages. Have you ever tried to read their brain?? Sometimes I feel that somewhere in their heart they must be thinking to be students like us! , to have better life-style like that of students....but...when the ridiculous foods make me their victim I develop mild amnesia.

They don’t expect anything from us, because they know that we are useless in the field of social service. That is the bitter truth as we write articles about them during our free hours & do nothing after that. Even they don’t that the govt. has banned the use of children under14 years of age in any work. I know that we are not in a stage to help them...but we can definitely give them 1 thing that is ‘Good behaviour  & a sweet smile’.....these hostel workers are just like bricks in a large wall....so I request all my friends to behave sweetly with these unfortunates, so that they can blossom some smiles on their tired looking face...............

BED NO -16

24th April 2010, 9.30 AM: Cricket god Sachin was celebrating his 37th birthday, Sania Mirza was busy impressing her in-laws in Pakistan, Lalit Modi, the father of IPL was planning to remove all the stains of corruption from his professional life &unfortunately we were having our 8th semester paediatric clinical examination. Although it is my favourite subject, I have used the word ‘unfortunately’ because if you don’t hate exams as a student, then you are simply ‘ABNORMAL’.

In order to show some respect to my favourite subject, I spent a lot of time in library on the day before exam &  you know, ‘the more you work, the more you expect’(sorry for going against Buddhism). Hence, since morning I was praying for a simple & uncomplicated case. Luckily I got a simple but difficult case. Due to shortage of patients, a single case was given to two students. So I was there with Rakesh (my next roll no). It does not matter what exam you are appearing, you are bound to feel the nerves & we’re not the lucky exceptions. With the adrenaline rush & the mercury kissing the 40 degree mark, we started our clinical history taking.

Bed no -16. Oh my god, there were 3 babies, but it was not difficult to identify the patient. He was a 2 year old boy fighting against Mr Death. Ooh, how can I explain the boy’s condition? It is easy to write a medical case in an exam than to type it in the MS words. In fact there was nothing good to explain. He was severely malnourished, all the ribs were visible to the naked eye, all the hairs were depigmented enough to snatch your attention, all the skin infections would’ve forced anybody to tell ‘oh my god !!’. His body was all about infected skin & bones .Please don’t ask about muscles!!!! Even the amount of dresses of a porn-star can outnumber his muscle layers. He was the 4th child of her mother; his father had expired 1 year ago, both of his elder sisters were playing on the bed, his only brother was a rickshaw puller. All of them were illiterate; her mother was a daily laborer & the only earning member of the family!! It was really a herculean task for us to carry out the clinical examinations because of his skin conditions. It was/is really painful to see a baby being admitted to hospital just because he doesn’t have enough to eat, just because he is poor, just because his father passed away after just throwing his sperms for reproduction of 4 babies!!!?? It was really hard to concentrate after watching the poor baby. I expressed all my feelings to Rakesh & then he put his hand around my shoulder & told that ‘medical profession is all about maintaining your cool in irritating atmospheres’...I was really surprised to hear such a matured comment from him & it was similar to Kasab getting a clean-chit from government of India. Being reminded by Rakesh about our profession, we completed our clinical exam & waited our turn for the bed-side viva-voce.

Usually we get a lot of time & scope to refer books during any clinical exam. So we didn’t waste any time & started to study a-z about our case ‘MARASMUS’. My brain was playing hide & seek with GHAI uncle(Dr O.P Ghai is the author of the ped.book), but my heart was there with the poor family. Only two questions were striking my grey matter-1) what are the clinical features & management of a case of MARASMUS ?? & 2) How can the baby get some help??? Nobody was giving proper attention to bed no-16 (just because they were poor & illiterate !!?). The PGs were behaving rudely!! Then came a person at around 12 noon with some medicines & a mat & asked me & Rakesh about our patient’s condition. We explained him the current status of the baby. I don’t know why I didn’t enquire about the person, but after a minute of silence he told that ‘I don’t have any relation with the patient’s family, but I am helping them just because of the innocent baby, I am doing it for the sinless face, for a family which has nothing to lose.’ That was a heart-touching moment for me. It was really refreshing to have such kind of persons in one of the poorest states of India. Both of us were highly impressed by that black skinned person with the glow of sympathy on his face. After that I asked myself ‘Can you do that in future?’ &&&&&………………………………….
Before I could answer myself, the examiner came to us started the legal form of academic ragging….

Its evening & I had already tried so many things to entertain myself. But that bed no-16 was not allowing my mind, body & soul to escape.  That is why I started writing about this case. This case is just like a drop from an ocean. It is really pathetic to see people not having one of the basic needs ‘FOOD’. All of us are aiming for a good PG seat (a good company job in case you are a B-tech student), an expensive car, a beautiful wife/husband, salary good enough to choke a bullock etc. Have you ever thought about these people who are facing death due to protein-energy malnutrition?? If not, please start to think. Please make your own plans to help these people in future. All of us will be successful in life. But if we can share our success with these children with the token of HELP, then only we can say that ‘Yes, now we have achieved something’. So please keep some space in your dreams to help these people. Pleaseeeeeeeeeeeeee.
